NSF for LTnC - Mid Point Review

2010 will be five years since the Long Term (Neurological) Conditions National Service Framework was published.  The Long Term Conditions Delivery Support Team wish to gain the support of stakeholders to develop a Mid Point Review.  

An on-line survey was made available to gather as many opinions as possible on what a review should attempt to cover. The results of the survey are now available to download (in the documents section of this page).

I have just been informed about this survey and appear to have missed the deadline to complete.  Is it possible still to do so?
