South East Coast Regional Updates:

July 2010:

The SHA LTnC Network continue to support the implementation of Care and Support Planning. Local meeting have taken place with within the PCTs to plan and launch local initiatives and these are supported by regionally arranged events.

The Innovations Project on the implementation of care and support planning systems into the everyday routines of GP practices is now under way in three practices in three PCTs across the Region. This projects seeks to highlight potential challenges to the implementation of Care and Support Plans and identify potential mechanisms to overcome those challenges.

The SHA LTnC Network has led on the wider learning and implications of the outcomes from the Regions Personal Health Budget pilots. Facilitators for a Region wide Learning Project have been appointed to lead a number of learning sets throughout the Region. These learning sets will commence during the late summer.

The Region is developing a Peer Support Project which will look outwards for peer support for LTnC commissioners. This project has been well received within the Region and expressions of interest have been received from around the county.

In the national arena the LTnC Delivery Support Team member for the Region is involved in the LTnC Mid Point Review. A timetable for the completion and formal piloting of the Commissioners and Users Questionnaires has now been finalised. Similarly a national project to develop Quality Indicators for Neurological Condition has been ongoing in collaboration with the Neurological Alliance and the programme timetable is set for delivery of Indicators by end of 2010.
