I am the South Central regional lead for the LTC Programme and have held a variety of NHS management and development roles in southern England for 25 years. This has included working in close partnership with local government and third sector colleagues in Bristol to commission services to promote older people’s independence and well-being, such as home-based intermediate care and rehabilitation.
During the last 5 years I have managed programmes to implement national policy for older people, continuing healthcare and diabetes within a Strategic Health Authority (SHA), working closely with PCT, social care and third sector colleagues to support sustained improvement through better commissioning.
I joined the LTC Delivery Support Team in May 2009 and am committed to the development of joined-up services across health and social care. I work closely with SHA colleagues to support PCTs, social care and the third sector to develop person-centred services for people with long term conditions in the South Central area.
Commissioning for Chronic pain: November 2009 conference follow-up and QIPP – the report on last November’s Conference has now been finalised for circulation to colleagues nationally (SHAs, PCTs and trusts). A copy of the report has been shared with the national QIPP team to highlight the role of self care in managing chronic pain. Links have also been established to work on persistent pain and public health currently being undertaken by DH colleagues.
The report from the LTC organised national pain conference 'whose pain is it anyway?' is available to download, along with the presentations from the day.
Participated in South Central SHA Masterclass on Personal Care Planning with SHA, PCT, community provider and practice-based colleagues.
Submitted an Outline Business Care for piloting Personal Care Plans in a prison setting to Offender Health for funding. A decision is still pending subject to the availability of resources for prison health in the current year, which has still to be clarified.
LT Neurological Conditions NSF – working with PCTs, developed a high-level self-assessment of each Local Implementation Group’s progress against the NSF Quality Requirements. The NSF was used as a marker to inform the SHA Clinical Executive Group on progress with implementing its LTC Programme.
Attending NSF Local Implementation Groups and discussing options for shared learning and networking across the region. The prevailing view is for sharing information electronically via the NHSSC site on the LTC community website and this is currently being progressed.
Began preparatory work with the PCT selected to participate in the national Mid Point Review pilot on behalf of NHS South Central.
Continuing to support Local Implementation Groups and Neurological Alliances as requested with NSF implementation. Working with a LIG to facilitate decisions and action plans in relation to future community service models for people with neurological Conditions.
With SHA and PCT colleagues from NHS South Central, participated in the QIPP ignition event on 30 June 2010 and contributed to the development of QIPP plans, including those in one locality which relate to improving pain services.
