Anna Baker
I am the East Midlands regional lead for the LTC team, and commenced this post in May 2010. The central aim of this post is to support all organisations across the area to implement the recommendations of the NSF for Long Term Neurological Conditions. I am looking forward to working with health, social care and third sector organisations, in partnership with patients and carers, to support the planning and development of services.
My background is nursing, midwifery, health visiting, and management and I have worked in a variety of settings, both in UK and abroad, including Birmingham, Scotland, Germany, Hampshire and Northern Ireland. I have experience of service development, project management, procurement, operational management and partnership working with patients and carers.
Since 2005 I have been in management roles predominantly for adult community services, and latterly for staff who manage patients with long term conditions across Derbyshire.
I have been in post since mid May and during the induction period have met key people in health, social care and third sector, including those from the VOWNNET group (Epilepsy, Parkinson’s disease, Multiple Sclerosis, Motor Neurone Disease, Progressive Supra Nuclear Palsy, Tuberous Sclerosis, Headway and Muscular Dystrophy).
I am supported by the Long Term Conditions lead from the Strategic Health Authority (Lorraine Wright), and by Ian Salt (Programme Manager for our team), and the initial focus of ny secondment is on supporting all parties around the implementation of Personal Care Plans (PCP). This by necessity will be partnership approach with Social Care and third sector colleagues, and tie in with Direct Payments and Personal Health Budget agenda, and the self care, partnership approach and empowerment of those with long term conditions.
An initial action will be supporting the planning and co-ordination of an East Midland stakeholder event on 20th of October, with the following outcomes:
I have invited representatives from Primary Care Trust and Practice Based Commissioners, Social Care, Third Sector and patient representatives to look at sharing current good practice, demonstrating existing tools and networking.
Outcomes from this event will be available on the website.
Another priority area will be to complete the work with the Joint Epilepsy Council in the development of a document to offer guidance to commissioners on developing business plans for Specialist Nursing posts.
